This is very common. It actually happened to me. Thankfully I did not catch anything but my doctor said hundreds of his patients who went to that clinic for endoscopy did.
Boiling them alive is fitting punishment for crap like this. They do it knowingly, to save costs. Some stuff is supposed to be thrown out and is fairly expensive.
